Patient Access Services would like to welcome you to Knox Community Hospital. We provide all registration, scheduling, outpatient pre-certification, and hospital switchboard services. Our mission is a commitment to creating and maintaining a courteous, warm, and empathetic environment for all customers.
It is the policy of Knox Community Hospital to admit and treat all persons without regard to race, color, sex, handicap, national origin, or religious creed. Admission requirements and assignments of hospital facilities are the same for all customers.

| For your convenience, you can complete and submit an on-line pre-registration form before your date of service. Pre-registering on-line will help speed processing on your date of service. |

Central Scheduling Information
When your physician schedules your appointment, certain information is required in order for us to begin your registration process. This information is discussed below. If you have questions or require assistance, please feel free to contact our scheduling department at (740) 392-TEST [392-8378]. We are available Monday through Friday, 7:00am – 5:30pm. Voice mail is available after-hours; let us know when it is convenient to return your call.
All information is kept confidential and will be verified upon your arrival to the hospital.
Outpatient Registration
When your physician gives you an order to receive outpatient testing or schedules you for testing, please report to Central Registration. We are located in the center of the first floor. Our hours are Monday through Friday, 6:00am – 4:30pm. If we are closed, Emergency Registration will serve your outpatient registration needs. Emergency Registration is located at the back of the hospital. Please arrive at least 30 minutes prior to your scheduled appointment. This time allows us to finish serving customers who arrive before you and to give you the time necessary to complete all required paperwork prior to your service.
Pre-registration expedites your check in process on your appointment day. Please arrive at Central Registration 15 minutes prior to your appointment time. There may be a few additional questions we need to clarify and we will ask you to sign a consent form. Please bring your physician's order and all insurance cards with you. When You Arrive for Outpatient or Admission Services
Our designated customer parking for outpatient and admission services is located in the front of our facility. When entering the front door, continue straight down the hall to Central Registration. Sign in at the semi-circular desk and we will serve you as soon as possible. Our policy is to serve customers on a first-come, first-serve basis. However, the physical and psychological needs of each patient will be evaluated and our process will be modified appropriately to adjust to those needs. We appreciate your patience during high volume times.
When coming for outpatient testing you must bring your doctor’s written order for treatment. To eliminate potential delay in service, please review it for accuracy prior to arrival. The Federal Government states that a complete physician’s order must include the following elements:

Medicare requires us to verify medical necessity for certain outpatient testing. If your testing does not meet Medicare’s guidelines, you will be asked to sign an Advanced Beneficiary Notice stating that you will be financially responsible for the service.
Insurance
It is important that you bring all insurance cards and proper identification with you in order to promptly complete the registration process. It is your responsibility to obtain any pre-admission or pre-testing authorizations that may be required by your insurance company.
To expedite account resolution, we will be happy to collect any and all deductibles, coinsurance, and non-covered charges at the time of registration.
What to Bring
If you have an advance directive and/or living will, please bring a copy with you for your hospital records. We ask that you bring a minimum of personal belongings with you. Please leave any valuables at home. The hospital is not responsible for loss or damage to personal property. Please bring a current list of medications with you including the name of the medication, dosage, and prescribed instructions for taking your medication.
If you are being admitted to the hospital and will be staying overnight, it is suggested that you bring a robe, slippers, sleepwear, toilet articles, and any personal items for your comfort.
When coming for outpatient testing you must bring your doctor’s written order for treatment. Please review it for accuracy prior to arrival.
Instructions
If you are coming to the hospital for outpatient tests or surgery, be sure to follow any instructions given to you by your physician, i.e., no eating or drinking after midnight before the day of the procedure. If you are having a surgical procedure, you must arrange for a ride home. In the interests of safety, the hospital does not release patients to leave alone, and you should not drive on the day of a surgical procedure.
When You Arrive for Emergency Services
Our designated customer parking for emergency services is located at the rear of our facility. Check in at the registration desk and a staff member will initiate your service. Our policy is to serve customers on a first-come, first-serve basis. However, the physical and psychological needs of each patient will be evaluated and our process will be modified appropriately to adjust to those needs. We appreciate your patience during high volume times.
What is Registration?
To expedite your billing process, we will verify key demographic and insurance information. You will be asked to show all insurance cards and sign a consent form during your registration process. We are required to ask you about Advanced Directives and religious preference. If you are a Medicare patient, there is a Medicare Secondary Payer questionnaire that we are required to complete, as well. The average registration takes 6 minutes.
Switchboard
All calls into the hospital are answered on a first-come, first-serve basis. Your call will be received by a professional operator 24 hours a day, 7 days a week. To protect the privacy of all customers, we are unable to page patients or family members at any time. |
