Our Patient Representative
Evelyn Chalfant, RN, our patient representative, serves as a patient advocate, working with patients, our hospital administration and staff on behalf of patients and their families. She is able to share information and address any concerns with the utmost respect and confidentiality. If you have any concerns about your visit at Knox Community Hospital, please contact Evelyn at 393.9890 or 393.9891. Her normal hours are Monday through Friday from 7 am — 3:30 pm; after hours, please leave a message, and Evelyn will return your call as soon as possible.
If you have safety or quality of care concerns at Knox Community Hospital that have not been addressed by the hospital, please contact the Joint Commission on Accreditation of Healthcare Facilities (JCAHO) via their website at www.jcaho.org or by calling 630.792.5000.
